We are committed to providing you with the best possible care. If you have dental insurance, we are anxious to help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance, and your understanding of our payment policy.
Payment for services is due at the time services are rendered unless written payment arrangements have been approved in advance by our office manager.
We realize that temporary financial problems may affect timely payment of your account. If such problems do arise, we encourage you to contact us promptly for assistance in the management of your account. Returned checks and balances older than 30 days will be subject to additional collection fees and interest charges of 1-1/2% per month. Charges may also be made for broken appointments and appointments canceled without 48 hours advance notice. Furthermore, in the event of enforced collections, you will be responsible for legal or collection charges of the higher of thirty percent (30%) or $250.00 attorney fees plus expenses and court costs.

As a courtesy to you, our patient, we will accept assignment of dental insurance benefits, and we will be happy to process your claim form for reimbursement. However 1/3 of your estimated co-payment is due at the time of scheduling and the final portion of your co-payment is due at the time of services. Any amount unpaid by your insurance company will also be your responsibility.
We will gladly discuss your proposed treatment and answer any questions relating to your insurance.
You must realize, however, that:
- Your insurance is a contract between you, your employer and the insurance company. We are not a party to that contract.
- Our fees are generally considered to fall within the acceptable range by most companies, and therefore are covered up to the maximum allowance determined by each carrier. This statement does not apply to companies that reimburse based on an arbitrary “schedule” of fees, which bears no relationship to the current standard and cost of care in this area.
We must emphasize that, as dental care providers, our relationship is with you; not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date that services are rendered.
In an effort to provide the best possible service for our patients, we have implemented a new Appointment Reservation Policy.
We reserve quality time for each patient in our practice. An appointment with your name on it is a bond of trust that we will be here to serve you, and you will be present for that reserved time. Our policy in this regard is extremely firm. We realize the value of your time and ask that you respect our time.
Whenever an appointment is reserved for subsequent dental procedures, we will request a 1/3 non-refundable down payment ($25.00 minimum) of the anticipated fee. This ensures that the necessary time slots will be reserved for you. However, we recognize that emergencies do arise. If you advise us at least 48 hours in advance, we will gladly reschedule your treatment with no additional fee.
